Case management is a care delivery system which is currently surfing on a wave of popularity as a result of its combined efficiency and effectiveness. Most health care institutions in Quebec have implemented a case management model. Case management is an integrated, complex system which is multidisciplinary by nature and which requires numerous tools to be effective.
